Job summary

CareSource is seeking a Team Lead, Community Based Care Management to provide frontline management and support for the Care Model in alignment with corporate goals. You will supervise staff, recruit and train the team, and coordinate services with community partners.

The role requires RN/BSW/MS qualifications and 3+ years of healthcare experience, with preference for managed care/case management and supervisory experience. This mobile role involves travel and salary range listed below.

Qualifications

• RN, BS in Social Work or MS in a relevant field is required.
• Minimum 3 years of healthcare experience is required.
• Minimum 2 years in managed care/case management preferred.
• Supervisory experience is preferred.

Responsibilities

• Hands-on supervision of staff and day-to-day operations.
• Recruit, hire, onboard, and train departmental team members.
• Train and support staff for effective CM program administration.
• Prepare and use reports to monitor systems and service quality.
• Ensure SOPs are current and standardized to meet business needs.
• Assist in development of new projects and processes.
• Oversee documentation within defined protocols and guidelines.
• Provide technical/content support aligning programs with strategic plans.
• Collaborate with internal resources to monitor service levels and analyze trends.
• Coordinate services with community partners to maximize resources.
