Qualifications

• Pharmacist
• Masters Degree, or Doctorate Degree
• No Travel Required

Responsabilités

• Leads committees responsible for establishing and implementing medication-related policies and procedures (e.g. Pharmacy & Therapeutics, Medication Safety, Antimicrobial Management Program)
• Direction of one or more service lines with respect to pharmacy service lines (e.g. cardiovascular, pediatrics, critical care)
• Demonstrates leadership within the pharmacy department through a record of improvements in pharmacy practice
• Advances the profession in the area of pharmacy practice in which they serve
• Provides regular, formal education to pharmacy and interdisciplinary teams
• Mentors pharmacy students, residents, and colleagues in professional pharmaceutical care
• Performs all responsibilities of the clinical pharmacist as outlined below
• Verifies CPOE orders for accuracy and intervenes when orders may be unsafe or inappropriate
• Interprets medication orders, documents in patient records, and troubleshoots COE orders as appropriate
• Prepares and dispenses medications including sterile, chemotherapy, and PN preparations safely using USP 797 & 800 guidelines
• Evaluates TPN for appropriateness based on facility policies or ASPEN guidelines
• Issues controlled substances to patient care areas and maintains records as required by law
• Ensures safe, evidence-based, and cost-effective medication therapies
• Monitors therapy regimens for contraindications, interactions, allergies, and dosing appropriateness
