Job summary

Metropolitan Ministries in Tampa, FL seeks an experienced Lead Case Manager to provide in-office case-management to families in the emergency shelter program. You will evaluate needs, connect clients with services, and ensure high-quality data in HMIS Unity during entry and exit.

You will also supervise interns, support monthly reporting, coordinate Sanctuary groups, and participate in staff trainings and collaborations to advance the organization's mission of helping the homeless and at-risk

Qualifications

• Bachelor’s degree in Social Work or Human Services field preferred.
• 3-5 years of Case Management experience.
• Knowledgeable about homeless issues and mental health/substance abuse.
• Willing to pursue continuing education and licensure opportunities.

Responsibilities

• Provide weekly in-office case-management services to families in the Emergency Shelter Program.
• Develop client support plans, goals, and resources; maintain case notes and data in HMIS Unity.
• Oversee data quality of client files at entry and exit; coordinate Sanctuary groups.
• Monitor monthly entry follow-ups and ensure updates within the first week of each month.
• Support quality assurance in residential program services and participate in meetings.
• Maintain professional boundaries and confidential client records; generate outcomes reports for funders.
