Job summary

Avenues Home Care is seeking a Lead Caregiver to provide compassionate in-home care to elderly, disabled and vulnerable clients in the Nashville area. You will perform ADLs, assist with medications, light housekeeping, meal prep and companionship, following each client's plan of care.

We value safety, reliability and strong communication. You will report daily activities, maintain confidentiality, and participate in ongoing training and HIPAA-compliant documentation.

Qualifications

• CNA license or equivalent preferred.
• CPR & First Aid certifications preferred.
• Strong communication and reliability required.
• Ability to lift up to 50 pounds as needed.

Responsibilities

• Assist with Activities of Daily Living (ADLs) per client plan.
• Assist with medication support, light housekeeping, meals and transportation.
• Provide companionship and monitor client condition.
• Report daily activities to supervisor using agency systems and logs.
