Job summary

Avenues Home Care in Nashville, TN is seeking an In Home Caregiver to provide compassionate support to elderly and other clients in their homes. You will assist with ADLs, medication reminders, meal prep, light housekeeping and transportation as needed, always prioritizing safety and dignity.

You will report daily activities to your supervisor and follow the client’s care plan using our management system. The role requires good communication, reliability, and the ability to lift up to 50 pounds.

Qualifications

• Assist with ADLs per client care plan.
• Provide medication reminders and support.
• Assist with light housekeeping, meal prep and transportation as needed.

Responsibilities

• Assist with Activities of Daily Living as specified in care plan.
• Report daily activity to supervisor and log in agency app.
• Maintain safety and confidentiality; comply with HIPAA and company policies.
• Other duties as assigned by supervisor.
