Heritage Health Network (HHN) is seeking a Lead Care Manager to support members through our Enhanced Care Management (ECM) program in a hybrid role. The position blends two remote days with three days in the field across San Diego County, enabling strong, in-person connections.

You will coordinate care, develop personalized plans, and collaborate with healthcare teams and community resources to improve outcomes for our members.

Qualifications

• Experience in care or case management, preferably in community settings.
• Knowledge of Medi-Cal and Enhanced Care Management (ECM) is strongly preferred.
• Excellent written and verbal communication and interpersonal skills.
• Ability to collaborate in a team-oriented environment.
• Willingness to travel within San Diego County.
• Hybrid role with two remote days and field work.
