Heritage Health Network (HHN) seeks a Lead Care Manager to guide members in their Enhanced Care Management (ECM) journey. The hybrid role blends remote coordination with field work across Los Angeles County, empowering you to build meaningful relationships and drive outcomes.

Formación

• Experience in care or case management (community-based preferred)

Lead Care Manager (ECM) - Hybrid Role

Join Heritage Health Network and make a meaningful impact in your community as a Lead Care Manager supporting members through our Enhanced Care Management (ECM) program.

This is a hybrid position (2 days remote, 3 days in the field), giving you the flexibility to work from home while building strong, in-person connections with members across Los Angeles County.
