Job summary

LifeSTEPS is a nonprofit organization providing supportive housing and services across California. We are seeking a Lead Care Manager to assist and aid members with enhanced coordination of care in Alameda County. This is a non-exempt, 20 hours/week position with remote home office and field locations.

The role involves care coordination, preparing care plans, documentation, and collaboration with healthcare providers to support housing stability for clients.

Qualifications

• Bachelor's degree in social work, psychology or related field.
• Two years of experience in a similar environment providing social services.

Responsibilities

• Promote and live out LifeSTEPS workplace values of integrity, innovation, empowerment, respect and personal growth.
• Assess members to identify needs across personal, financial, housing, and health aspects to determine assistance.
• Develop and monitor the member’s care plan and revise as needed.
• Coordinate with primary care providers, specialists, behavioral health, and community-based services involved in care.
• Coordinate with external entities providing member care and coordinate transportation and reminders for appointments.
• Advocate and refer members to community resources and document activities and follow-up.
