Qualifications

• Active FL Nursing License required.
• BLS required (AHA) within 30 days of hire.
• 3–5 years clinical acute care experience.
• Commitment to ongoing professional development as part of practice.

Responsibilities

• Direct patient care coordination and oversee interdisciplinary team to meet quality, satisfaction, financial and operational goals.
• Ensure compliance with Joint Commission and regulatory requirements within the department.
• Lead Interdisciplinary Care Coordination Team including pharmacists, social workers, physicians, home health agencies and SNFs across care continuum.
• Observe, coach and evaluate staff; participate in orientation and training.
• Collaborate with leadership on staffing, hiring decisions and staff evaluations.
