Job summary

UF Health in Florida seeks a Supervisor of Care Coordination to provide leadership in coordinating patient care within the Medical Surgical and Care Coordination Services. This role guides the interdisciplinary team, reconciles plans of care, and ensures efficient hand-offs across care settings.

The position requires graduation from an accredited School of Nursing, Florida RN license with BLS, and 3–5 years of acute care experience. A BSN is preferred or a commitment to obtain one.

Qualifications

• Graduate Accredited School of Nursing required.
• Active Florida Nursing License and BLS.
• 3 to 5 years Clinical Acute Care experience.
• BSN preferred; commitment to earn BSN if not possessed.

Responsibilities

• Coordinate and direct patient care using clinical and operational management to meet quality, patient satisfaction, financial, and operational goals.
• Ensure compliance with Joint Commission, federal, state and other regulatory requirements; implement standards of care in consultation with leadership.
• Lead the Interdisciplinary Care Coordination Team across the continuum of care to reconcile the Plan of Care and manage hand-offs.
• Observe and coach staff; evaluate educational needs; participate in orientation and on-the-job training.
• Collaborate with department leadership on hiring, termination, progressive discipline and staff evaluations.
