• Two years of experience with elderly or disabled adults.

Responsibilities

• Assess, develop and implement care plans with appropriate response plans.
• Assess and diagnose mental illness for participants.
• Provide support to social service department and guide treatment decisions.
• Deliver mental and behavioral therapy using strength-based approaches.
• Provide counseling to individuals or groups; offer referrals or peer support.
