Job summary

Dignity Health Connected Living in Shasta County seeks a compassionate Social Worker to support seniors and people with disabilities, guiding them through complex challenges.

You will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to community resources while collaborating with multidisciplinary teams.

Qualifications

• Bachelors degree in social work, psychology, counseling, rehabilitation, gerontology or sociology is required.
• One (1) year experience working with the elderly and individuals with disabilities is required.

Responsibilities

• Conducts in-depth psychosocial assessments/reassessments covering psycho-social, rehabilitation and environmental concerns.
• Refers clients to special consultants to provide detailed evaluations and recommendations to improve the client's functional level.
• Develops the care plan, implements the services and conducts follow-up and monitoring of the plan.
• Identifies and develops support systems for the client.
• Consults with the Health Practitioner.
• Performs case recording and reporting.
