Dignity Health’s Glendale Memorial Hospital and Health Center seeks a compassionate Social Worker to provide psychosocial assessments, care planning, counseling and crisis intervention across inpatient, outpatient and emergency settings.

You will collaborate with a multidisciplinary health team, advocate for patients and families, coordinate resources, and support transition planning and post-acute placements.

Formación

• Two years of clinical and/or medical social work experience in a medical facility is required.
• Master's Degree from a CSWE-accredited School of Social Work.
• Knowledge of health care theories, end-of-life dynamics, grief/bereavement, substance abuse identification/interventions, and community resources.

Responsabilidades

• Perform psychosocial assessments and develop care plans.
• Provide counseling and crisis intervention; advocate for clients.
• Coordinate with multidisciplinary teams to deliver holistic care.
• Connect clients to community resources and support post-acute placement.
