Job summary

CommonSpirit Health's Dignity Health Connected Living in California is seeking a Social Worker to advocate for individuals and families facing complex challenges, providing counseling, crisis intervention, and linkage to community resources. This is a full-time position within a compassionate program focused on elder and disability care.

In this role you will conduct psychosocial assessments, develop care plans, collaborate with multidisciplinary teams, document cases, and help build support

Qualifications

• Bachelor's degree required in relevant field (social work, psychology, counseling, rehabilitation, gerontology or sociology).
• Minimum 1 year experience working with elderly and individuals with disabilities.

Responsibilities

• Conduct in-depth psychosocial assessments and reassessments covering psycho-social, rehabilitation and environmental concerns.
• Refer clients to special consultants to provide detailed evaluations and recommendations to improve the client's functional level.
• Develop the care plan, implement the services and conduct follow-up and monitoring of the plan.
• Identify and develop support systems for the client.
• Consult with the Health Practitioner.
• Perform case recording and reporting.
