UT Southwestern Medical Center in Dallas seeks an experienced Medical Social Worker to provide advanced social services in a clinical and hospital setting, under moderate supervision. Requires a Master's in Social Work, LMSW or LCSW preferred, and at least 3 years of medical/psychiatric social work experience.

You will lead discharge planning, coordinate with case managers and community resources, document patient progress, and support families.

Qualifications

• MSW degree required.
• 3+ years of medical/psychiatric social work experience; 1 year as Social Worker II or equivalent.

Responsabilités

• Leads and coordinates social work interventions to support patient care and discharge planning with external case managers and community agencies.
• Develops and implements programs and services addressing psychological and social needs of patients and families.
• Provides counseling and creates ongoing social work treatment plans based on assessed needs.
• Maintains documentation, coordinates resources, and educates families about community options and payor resources.
• Performs duties aligned with direct patient care, records management, and research participation as applicable.
