Job summary

• 3 years in medical/psychiatric social case work; 1 year as Social Worker II.
• Licenses: LMSW or LCSW may be required.
• Knowledge of discharge planning and patient care coordination.

Responsibilities

• Leads and integrates social work functions and discharge planning with external case managers.
• Identifies, develops, and implements programs for psychological and social needs of patients and families.
• Assesses social service needs; provides counseling and coordinates effective treatment plans.
• Plans development of outreach services by collaborating with physicians, health care providers, and community professionals.
• Educates families and patients about community resources and health care issues.
• Provides discharge planning referrals and continuity of care.
• Maintains communications with community resources and agencies to assist meeting patient needs.
• Maintains patient records and case histories for each patient.
• May participate in clinical or laboratory research in related areas.
• Performs other duties as assigned.
