UT Southwestern Medical Center in Dallas, TX seeks an Inpatient Rehab Social Worker (LMSW) to join a multidisciplinary team. The role supports discharge planning, coordinates resources for patients, and facilitates psychosocial care for patients and families within an acute rehabilitation setting.

Graduate with CSWE-accredited MSW and licensure as LMSW; BLS is required. Two years of related experience is preferred.

Qualificações

• Master's degree in Social Work from a CSWE-accredited program.
• LMSW license issued by the Texas State Board of Social Work Examiners; temporary license acceptable during first 90 days.
• BLS certification required, though not in Transplant Departments.
• Preferred: 2 years related experience.

Responsabilidades

• Participates in multidisciplinary rounds and determines patient discharge needs (transportation, DME, LTAC, SNF, home health, hospice).
• Uses crisis intervention and problem-solving to support patient and family psychosocial needs.
• Maintains documentation to meet department and UT Southwestern standards.
• Identifies and reports required cases to governmental agencies as needed.
