Qualifications

• Master's degree in Social Work from a CSWE-accredited program.
• LMSW license issued by the Texas State Board of Social Work Examiners; temporary license acceptable during first 90 days; CPI training within 30 days of hire.

Responsibilities

• Participates in multidisciplinary rounds and determines patient needs for discharge transitions (e.g., transportation, DME, LTAC, SNF, home health, hospice).
• Uses crisis intervention and problem-solving to support acute and ongoing medical and psychosocial needs of patients and families.
• Maintains documentation and referral information per department standards and UT Southwestern policies.
• Identifies and reports legally required situations to appropriate governmental agencies.
• Acts as an active member of the health care team, communicating with providers, patients, families, and staff.
• Pursues professional growth opportunities and provides education to staff, patients, and community resources.
