Job summary
Concierge Home Care in Okeechobee, FL is seeking a Licensed Clinical Social Worker (LCSW) to provide psychosocial support and behavioral health counseling to home health patients. Qualifications
- Active Florida Licensed Clinical Social Worker (LCSW) license.
- Minimum two years of clinical counseling experience.
- Home health or telehealth experience preferred.
- Valid driver’s license and reliable transportation.
Responsibilities
- Provide psychosocial assessments, counseling, and behavioral health support.
- Develop and coordinate care plans addressing social, emotional, and environmental needs.
- Connect patients with community resources and support services.
- Assist patients and families in navigating social, financial, and healthcare challenges.
- Collaborate with physicians and care teams to support patient outcomes.
- Maintain accurate documentation per Medicare guidelines.
- Document visits using AI-assisted EMR tools.
