Mobile Medical in Jacksonville, FL is seeking a Licensed Clinical Social Worker (LCSW) to provide psychosocial support and behavioral health counseling to home health patients. Required Florida LCSW license, minimum two years of clinical counseling, and preferred home health or telehealth experience; valid
Qualifications
- Active Florida LCSW license required.
- Minimum two years of clinical counseling experience.
- Home health or telehealth experience preferred.
- Valid driver's license, reliable transportation.
Responsibilities
- Provide psychosocial assessments, counseling, and behavioral health support
- Develop and coordinate care plans to address social, emotional, and environmental needs
- Connect patients with community resources and support services
- Assist patients and families in navigating social, financial, and healthcare challenges
- Collaborate with physicians and interdisciplinary care teams to support patient outcomes
- Educate patients and caregivers on coping strategies and available resources
- Maintain accurate and timely documentation in accordance with Medicare guidelines
- Document visits efficiently using AI-assisted EMR tools
