Job summary

Rehab Without Walls Neuro Rehabilitation in Corpus Christi, TX is seeking an LCSW to join our neurorehabilitation program. This role focuses on delivering goal‑driven social work care within patient homes and community settings, promoting functional independence in daily living activities.

You will collaborate with an interdisciplinary team while maintaining clinical autonomy, conducting comprehensive evaluations, and setting measurable objectives for holistic recovery in adult populations.

Qualifications

• Master's Degree in Clinical Social Work (LCSW) required.
• Current Licensed Clinical Social Worker Licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals with brain/spinal injuries.
• BLS First Aid and CPR certification required.
• Ability to lift 50 pounds and move patients with safety procedures (up to 100 pounds)
• Fine motor skills, visual acuity, and standing/walking for extended periods.

Responsibilities

• Performs comprehensive evaluations of patients and families.
• Sets measurable objectives with rehab team.
• Assists health team in understanding social and emotional factors affecting health.
• Instructs/counsels patients and families on coping with illness impacts.
• Participates in care coordination and resource identification.
• Identifies and helps access community resources to meet objectives.
