Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a Licensed Clinical Social Worker in San Antonio, TX to join our Home & Community Rehabilitation team. You will perform evaluations, design goals with the team, and support patients and families facing neurological injuries—stroke, TBI, spinal cord injury—and coordinate care.

Requirements include an MSW and active LCSW licensure; 1+ year rehab experience preferred.

Qualifications

• Master's in Clinical Social Work and LCSW licensure required.
• Minimum 1 year rehab experience with neurological injuries preferred.
• Ability to promote emotional and social adjustment in patients and families.

Responsibilities

• Performs comprehensive evaluations of social, emotional and support needs.
• Sets measurable objectives with the rehab team.
• Assists health team in understanding social factors related to health problems.
• Instructs patients and families in coping with illness-related dynamics.
• Participates in care coordination and identifies community resources.
• Aids patient/family in utilizing resources to meet objectives.
