Job summary

Senior Community Care of Northern Kentucky PACE in Florence, KY is seeking a licensed clinical social worker to join our interdisciplinary team. This full-time role provides direct resident care, care planning, and collaboration with the IDT to deliver compassionate, person-centered services.

The position offers a 65,000–80,000 salary, Monday–Friday, 8:00am–5:00pm with occasional evenings and weekends, plus a full benefits package and paid time off.

Qualifications

• Current Licensed Clinical Social Worker licensure in KY.
• Masters degree in social work from an accredited program.
• Strong communication and customer service skills.
• Must pass background check and immunization requirements.
• Driver's license and transportation.

Responsibilities

• Serve as a contributing member to the PACE Interdisciplinary Team (IDT).
• Provide direct, hands-on clinical care to residents and assist with activities of daily living.
• Support residents’ well-being through compassionate, person-centered care.
• Monitor changes in residents’ condition and report to the nursing team.
• Maintain a clean, safe environment following care plans and safety protocols.
