Job summary

Senior Community Care of Northern Kentucky PACE in Florence, KY is seeking a dedicated Physical Therapist to join our team. This full-time role provides PT services to program participants at the PACE Center, SNF, and home settings, integrating therapy into the interdisciplinary plan of care.

The position offers a competitive salary range ($98,600–$106,400/year) based on experience, Monday–Friday hours (8:00am–5:00pm) with occasional evenings and weekends, and benefits including retirement plan,

Qualifications

• Graduate of a physical therapy curriculum. Current Physical Therapy license required.
• Minimum of one-year experience working with a frail or elderly population.
• Must have a valid driver’s license and means of transportation.
• Background check and immunization requirements.

Responsibilities

• Conduct initial comprehensive in-person physical therapy assessments and develop care plans.
• Perform periodic re-assessments and coordinate with the Interdisciplinary Team.
• Provide patient education, gait training, transfers, and in-home exercise programs.
• Supervise Physical Therapy Assistant and ensure quality of care and documentation.
