Qualifications

• Master's Degree in Social Work from an accredited program.
• Oregon LCSW license at hire or licensure in states served.
• Minimum 2 years of relevant social work experience.

Responsibilities

• Independently provide biopsychosocial/spiritual assessment, diagnosis, and treatment/interventions.
• Collaborate with patients and support systems to develop care plans with IDT.
• Adhere to NASW ethics and Standards of Practice while reducing health disparities.
