Qualificações

• Master's Degree in Social Work from CSWE-accredited School of Social Work, OR from a school in candidacy status by CSWE.
• Oregon Clinical Social Worker License at hire or License in states wherever care is delivered.
• 2 years relevant Social Work experience (may include CSWA).

Responsabilidades

• Conduct biopsychosocial/spiritual assessments and interventions.
• Collaborate with IDT to develop evidence-informed care plans across care continuum.
• Advocate for patients and reduce health disparities while upholding NASW standards.
