Job summary

Qualifications

Responsibilities

• Facilitates discharge planning from admission and assesses needs to prevent delays.
• Active participation in IPOC with Case Manager, Nurses, and Physicians.
• Prepares psychosocial evaluations and care plans age-appropriate to patient/family needs.
• Assesses psychosocial needs of patients, caregivers, and families including history and risk factors.
• Develops discharge plans and makes referrals to community services in a timely manner.
• Maintains accurate records of all social work activities in the patient’s chart.
