Job summary

Humboldt Park Health in Chicago, IL is seeking a Licensed Social Worker - Care Management for a full-time on-site role. You will conduct psychosocial assessments, develop discharge plans, and coordinate community referrals to support patient outcomes.

Qualifications

• 1–3 years of experience in a similar role required.
• 3+ years of experience in a similar role preferred.
• Master's degree in Social Work required.

Responsibilities

• Facilitates discharge planning from admission. Assess needs and prevent delays in discharge.
• Active participation in IPOC (Interdisciplinary rounds) with Case Manager, Nurses, and Physicians.
• Prepares psychosocial evaluations and care plans age-appropriate to patient/family needs.
• Assesses and diagnoses psychosocial needs through history, medical/psychiatric, resources and ADLs.
• Develops discharge plans and makes referrals to community services in a timely manner.
• Maintains up-to-date records of social work activities in patient charts.
