Responsibilities

• Demonstrates critical thinking skills appropriate to patient’s level of care.
• Collects and documents patient health data systematically and timely.
• Conducts thorough assessments of patients, analyzes data and develops care plans.
• Implements interventions from the plan of care safely and documents actions.
• Collaborates to promote timely patient placement by following hospital policy.
• Participates in peer interviewing and peer review processes as needed.
• Administers medications and treatments as prescribed, ensuring safety.
• Communicates effectively with patients, families, physicians, and staff.
• Educates patients and families on health conditions, procedures and care plans.
• Maintains compliance with regulatory standards and policies.
• Participates in quality, safety and effectiveness initiatives.
• Serves as a patient advocate and ensures rights are respected.
• Ensures safe handoff communication and documentation.
• Follows isolation guidelines and PPE usage; performs hand hygiene.
• Performs other duties as assigned.
