Job summary

The Christ Hospital Health Network is seeking an RN to provide patient care in a hospital setting, following the organization’s Professional Practice Model. The role emphasizes holistic assessment, care planning, and multidisciplinary collaboration to support patient and family-centered care.

Responsibilities include conducting thorough assessments, developing measurable care goals, coordinating discharge planning, and educating patients and families.

Qualifications

• The RN demonstrates the knowledge, abilities, and skills to provide age and culturally specific patient care and education.
• YEARS OF EXPERIENCE: None required.
• The RN must hold an Ohio RN license; BLS is required; ACLS within six months in appropriate units; BSN within five years of hire where applicable.

Responsibilities

• Assesses patient, family, and environment for safety risks and collaborates with the healthcare team.
• Prepares, coordinates, and revises care plans with measurable goals for each problem.
• Communicates plan of care using SBAR and handoffs with team members.
• Delivers patient education and family teaching in collaboration with the multidisciplinary team.
• Responds to emergencies and maintains safety and infection control practices.
• Evaluates nursing care effectiveness and continuity of care during handoffs and transfers.
