Qualifications

Responsibilities

• Assess physiologic health status and document care plans and outcomes.
• Collaborate with physicians and health team to coordinate treatments.
• Implement and evaluate the plan of care, ensuring safety and quality.
• Educate patients and families, discharge planning, and referrals as needed.
• Maintain accurate nursing documentation and participate in care conferences.
