Qualifications

Responsibilities

• Assesses patients' physiologic health status and conducts interviews.
• Assesses psychosocial health and identifies patient needs.
• Develops outcome statements and coordinated plan of care.
• Coordinates care with physicians and health team; documents plans.
• Maintains patient safety, administers treatments, and uses equipment correctly.
• Plans discharge and coordinates referrals; provides patient education.
