Job summary

SHPCA SCAN Health Plan seeks an experienced Registered Nurse to provide long-term care management for frail and diverse older adults, disabled clients, and caregivers. You will assess, plan, implement, and evaluate care, with emphasis on maintaining home-based care and preventing premature institutionalization.

The role requires travel across Los Angeles County, strong communication, and collaboration with physicians, home-health agencies, and community services.

Qualifications

• Must hold a California RN license in good standing.
• Nursing degree required; BSN preferred.
• 3+ years in health assessments, care planning, and documentation.
• Experience with home visits and older adults preferred.
• Willingness to travel 50%+ across Los Angeles County.
• Knowledge of elder care, environmental considerations, and intervention strategies.

Responsibilities

• Assess new clients for MSSP program eligibility and conduct assessments.
• Develop written care plans and discuss with clients or representatives.
• Educate non-clinical staff and present cases in team meetings.
• Coordinate with physicians, home-health, and community services to ensure safe, cost-effective care.
• Maintain timely documentation and service authorizations.
• Implement care plans and support census maintenance while delivering excellent service.
• Participate in QA reviews, audits, and trainings.
