Job summary

SCAN Health Plan is seeking a dedicated Care Planner to provide in‑home and telephonic care management for elderly and disabled adults, guiding them to stay safely at home and avoid premature institutionalization.

The role requires a Bachelor’s in Social Work (MSW preferred) and 1–2 years’ experience with older adults. Travel within the LA area is expected; bilingual English/Korean skills are required. Remote/field-based work is offered with competitive benefits.

Responsibilities

  • Conduct in‑home and telephonic assessments, care planning, and client monitoring through contacts with the client, professionals and the client’s support system.
  • Coordinate linkages with community‑based services and resources.
  • Maintain legal and clinical written records of each client’s case by completing all necessary documentation in a timely manner.
  • Ensure timely implementation of services for clients by completing all work within required timeframes.
  • Develop and implement a comprehensive care plan specific to client objectives and goals, with a measurable time frame and interventions.
  • Maintain ongoing documentation of contacts with client, family and caregivers, service providers, health professionals, and others connected to the client’s care.
  • Maintain effective communication with other disciplines and provide feedback on psychosocial aspects of the client’s situation.
  • Maintain accurate purchase of service records by completing monthly service authorizations and verifications to contracted vendors.
  • Keep current on Medicare and Medi‑Cal policies and related laws, participate in trainings, and support supervision.
  • Assist in census maintenance and participate in outreach activities.

Role Overview

Provide long‑term care management to frail, economically disadvantaged, culturally diverse older adults, disabled adults and caregivers to help them remain safely at home and to prevent premature institutionalization.

Responsibilities
  • Conduct in‑home and telephonic assessments, care planning, and client monitoring through contacts with the client, professionals and the client’s support system.
  • Coordinate linkages with community‑based services and resources.
  • Maintain legal and clinical written records of each client’s case by completing all necessary documentation (including assessment, care plans, service arrangements and client monitoring progress notes) in a timely manner.
  • Ensure timely implementation of services for clients by completing all work within required timeframes.
  • Develop and implement a comprehensive care plan specific to client objectives and goals, with a resolution plan within a measurable time frame and appropriate interventions outlined.
  • Maintain ongoing documentation of contacts with client, family and caregivers, service providers, health professionals, and others connected to the client’s care.
  • Maintain effective communication with other disciplines working with the client, provide feedback on the psychosocial aspects of the client’s situation and offer informal training to fellow care planners and staff in other disciplines.
  • Maintain accurate purchase of service records by completing accurate and timely monthly service authorizations and verifications to contracted vendors and following contract guidelines for service expenditures.
  • Increase knowledge of care planner functions by keeping current on Medicare and Medi‑Cal policies, Federal and State legislation affecting health and social services, maintaining updated community resource information, attending trainings as scheduled, actively participating in supervision, and striving to improve professionally.
  • Assist in maintaining census and provide excellent customer service in the community by participating in ongoing outreach activities.
  • Participate regularly in Case Conference, Quality Assurance chart reviews, completing other audits as required, helping to train new employees, participating in training sessions, and responding positively to suggestions for improvement.
  • Actively support the achievement of SCAN’s Vision and Goals.
Qualifications
  • Bachelor’s Degree in Social Work, Psychology, Gerontology or related field required.
  • Master’s Degree in Social Work (MSW) preferred.
Required Experience
  • 1–2 years experience working with the elderly.
  • 1+ year experience working in a hospital, home health agency or related setting in which the primary targeted population is older adults.
Bilingual Requirement

Bilingual in English/Korean (test to assess proficiency).

Travel & Transportation
  • Ability to travel within program’s catchment area and visits to client homes is required.
  • Experience performing home assessments, telephone monitoring, and travel to client homes throughout Los Angeles County.
  • Travel 50+% of the time in Los Angeles and surrounding areas.
  • Must have a valid driver’s license, automobile insurance and reliable transportation.
Additional Requirements
  • Knowledge of environmental concerns of older adults; theoretical understanding of the developmental needs for this population and means for intervention.
  • Strong organizational, verbal and written communication skills; demonstrated customer focus; and ability to work well with others.
  • Experience working in a case management setting, or demonstrated knowledge of case management techniques preferred.
