Job summary

SCAN seeks a bilingual English/Korean care planner to provide long-term care management for frail and diverse older adults, with a focus on helping clients stay safely at home. The role is remote/field-based and includes community outreach, assessment, care planning, and coordination with a wide network of services.

The ideal candidate holds a BA in social work (MSW preferred), 1–2 years serving the elderly, and is prepared to travel within Los Angeles County for client visits.

Qualifications

• Bachelor’s degree in social work, psychology, gerontology or related field required; MSW preferred.
• Bilingual English/Korean; proficiency tested.
• 1–2 years working with the elderly; travel in LA County required.

Responsibilities

• Conduct in‑home and telephonic assessments, care planning and client monitoring.
• Coordinate linkages with community services and resources.
• Maintain legal and clinical records with timely documentation.
• Ensure timely service delivery and adherence to care plans.
• Communicate with multidisciplinary teams and train staff as needed.
• Stay current on Medicare/Medi‑Cal policies and case management best practices.
