Job summary

SHPCA SCAN Health Plan seeks an experienced Registered Nurse to provide long-term care management for frail and diverse older adults, disabled clients, and caregivers. You will assess, plan, implement, and evaluate care, with emphasis on maintaining home-based care and preventing premature institutionalization.

The role requires travel across Los Angeles County, strong communication, and collaboration with physicians, home-health agencies, and community services.

Qualifications

  • Must hold a California RN license in good standing.
  • Nursing degree required; BSN preferred.
  • 3+ years in health assessments, care planning, and documentation.
  • Experience with home visits and older adults preferred.
  • Willingness to travel 50%+ across Los Angeles County.
  • Knowledge of elder care, environmental considerations, and intervention strategies.

Job description

Role Overview

Provide long‑term care management to frail, economically disadvantaged, culturally diverse older adults, disabled adults and caregivers to help them remain safely at home and prevent premature institutionalization.

Responsibilities
  • Assess new clients and determine appropriateness for the MSSP program, certify nursing‑home eligibility, and conduct assessments and reassessments for medical, physical, and psychological needs.
  • Develop written care plans approved by the program supervisor and discuss them verbally with clients or their representatives.
  • Provide ongoing health education to non‑clinical staff and present cases at weekly team case conferences, discussing complex health‑care issues for teaching purposes.
  • Contact clients’ physicians as needed to discuss identified health problems.
  • Educate clients or family members on self‑care, hygiene, and nutrition.
  • Collaborate with staff, physicians, home‑health agencies, community services, family members, hospital discharge planners, and other professionals to ensure appropriate services and a safe living environment at a cost below that of a skilled nursing facility.
  • Maintain monthly documentation of client contacts and other clinical records in a timely manner.
  • Complete accurate monthly service authorizations and verifications to contracted vendors, following contract guidelines for service expenditures.
  • Implement care plans for assigned caseload and support census maintenance while providing excellent customer service through ongoing outreach activities.
  • Participate in Quality Assurance chart reviews, audits, training sessions, and respond positively to suggestions for improvement.
  • Keep current on Medicare, Medi‑Cal policies, federal and state legislation affecting health and social services, update community resource information, attend trainings, actively participate in supervision, and pursue professional improvement.
  • Perform additional duties as assigned.
Qualifications
  • Registered Nurse with current and active California RN license in good standing.
  • BSN preferred; nursing degree required.
  • 3+ years of experience with health assessments, level‑of‑care criteria, care planning, and case documentation.
  • Experience working with older adults and conducting home visits highly desirable; experience with mental health and/or substance abuse issues is a plus.
  • Ability to travel throughout Los Angeles County and conduct home visits; 50%+ travel required.
  • Knowledge of environmental concerns, developmental needs, and intervention strategies for older adults.
  • Strong organizational, verbal, and written communication skills; keen attention to detail and customer focus.
  • Ability to prioritize multiple competing tasks, work independently and as part of a team, and thrive in a fast‑paced, dynamic environment.
  • Available to work Monday‑Friday, 8 a.m. to 5 p.m., with extended hours as needed.
  • Ability to travel 50%+ of the time, must have a valid driver’s license, automobile insurance, and reliable transportation.
  • Bilingual in English and Korean required (proficiency test will be administered).
Additional Requirements
  • Proof of Tuberculosis screening must be provided upon hire or a recent negative screening result.
