Job summary

NYAP in Columbus, Ohio is seeking a full-time Kinship Care Coordinator to join the Care Management Team. You will collaborate with Social Workers to provide supportive and educational services, conduct home studies, assess safety, and connect families with essential resources.

The role offers extensive training, loan repayment assistance, and a strong benefits package. Compensation ranges from $45,000 to $48,000 annually.

Qualifications

• Bachelor’s degree in Social Work or related Human Services field is required or preferred.
• Two years’ professional kinship care program experience is preferred.
• Excellent customer service and strong communication skills are essential.
• Comfort with independent work and teamwork within a collaborative care model.

Responsibilities

• Complete initial walkthroughs, home study assessments, background checks, and all required paperwork for Kinship Care placements.
• Educate kinship placements on reunification efforts and establish boundaries with parents.
• Develop safety plans and document all client and collateral communications and activities.
• Engage families, assess needs, provide education and resources, and build supportive relationships.
• Coordinate with the assigned Social Worker to develop and monitor service plans and progress toward goals.
• Conduct unannounced home visits to verify safety plans and services.
• Coordinate, organize, and order car seats and ensure safety for children.
• Collaborate with NYAP programs and staff across the organization.
