Job summary

National Youth Advocate Program seeks a Family Service Coordinator to provide in-home support, advocacy, and connections to community resources for youth and families in Terre Haute, IN. The role involves daily travel, collaboration with foster families, and alignment with case plans to promote safety and community integration.

You will coordinate services across health, education, and social sectors, complete documentation, and participate in supervision and CQI activities.

Qualifications

  • Bachelor's degree in Social Work or comparable Human Services field is required.
  • Minimum 2 years of direct service with youth and families preferred.
  • Must have flexible availability and willingness to work non-traditional hours.
  • Valid driver’s license, reliable transportation, and auto insurance required.

Responsibilities

  • Provide in-home support, transportation, parenting education, counseling, and advocacy.
  • Travel daily to deliver community-based services in line with contracts and regulations.
  • Complete all required documentation and trainings in a timely manner.
  • Coordinate and monitor services across mental health, medical, educational, and social services for youth and families.
  • Review and adjust youth plans to ensure progress toward goals.
  • Support family reunification and stabilization per the youth’s plan.
  • Provide safety assessments, crisis response, and behavioral stabilization as needed.
  • Participate in Continuous Quality Improvement and weekly supervision.

Family Service CoordinatorHours: M-F - 9am-5pm with non-traditional hours including evenings/weekends as needed.Compensation starting at $50,000.00Position SummaryUnder the general supervision of the Clinical Supervisor or designee, this position works to make a meaningful difference in the lives of the children and families served. This position works closely with youth, family, foster parents, and community partners, providing advocacy and support toward the ultimate goal of living safely and successfully in the community.Working At NYAPStudent Loan Repayment assistance, up to $1,200 per year!Up to 22 Days Off Each Year!Plus 11 Paid Holidays Per Year!Medical, Dental, and Vision for you and your family!Summer Hours Off (reduced work schedule)Competitive salaries and benefits including a 401(k)Tuition AssistanceWork Anniversary Trips!Peace LeaveParental LeaveMileage reimbursementResponsibilitiesActively supports, represents, and extends the mission, vision, and values of the organization.Provides in home supportive services, transportation, parenting education, counseling, community resource referral, advocacy, and other social service interventions to the youth and family, meeting or exceeding established outcomes thresholds on behalf of persons served.Travels daily, to provide community based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements.Completes all required documentation in a timely manner.Completes all required trainings in a timely manner.Coordinates and monitors services for the youth and family in the community, including but not limited to, mental health, medical, educational, psychological, vocational, and social services.Regularly reviews and modifies youth based plans to assure consistent progress and success on written plan goals.Establishes and maintains strong relationship with assigned foster parents and families of origin.Assists in family reunification and/or stabilization efforts with the youth’s primary family or other permanent living arrangement consistent with the youth’s case plan.Provides safety assessment, crisis response, and behavioral stabilization services as required to assure safety and stability for youth served.Consistently achieves established productivity thresholds.Participates in the Continuous Quality Improvement activities on a monthly/quarterly basis.Actively participates in the weekly supervision process.Minimum QualificationsBachelor's Degree in Social Work or comparable Human Services field from an accredited institution.2 years of work experience working in direct service with youth and families strongly preferred.Must have flexible availability as needed.Must have a valid driver’s license, reliable transportation, automotive insurance, and a good driving record.Must have car insurance requirements of 100,000/300,000 bodily injury insurance.A willingness to work flexible and non-traditional hours in the service of families of origin, and persons served.Capacity to be flexible and responsive to youth served, foster caregivers, system partners, and internal customers.Capacity to remain objective and professional in all areas of job function.Demonstrates tolerance and respect for the ideas and actions of others.Capacity to effectively work with and be respectful and sensitive to persons from various cultures, socioeconomic, ethnic, religious, and racial backgrounds.Proficient use of desktop and laptop computers, smartphones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.If this describes YOU, please apply today!www.nyap.org/employmentNYAP also requires all of our employees, regardless of their title/position, to hold and maintain automobile liability insurance coverage of $100,000/$300,000.
