Job summary

Point Quest Group is seeking a licensed Physical Therapist to provide school-based physical therapy services in a K-12 setting in Honokaa, Hawaii. You will conduct assessments, develop individualized plans, and deliver direct and consultative PT services to enhance mobility and educational participation.

You will collaborate with teachers, families, and multidisciplinary teams to support student outcomes, attend IEP meetings, and provide positioning and equipment recommendations as needed.

Qualifications

• Active Physical Therapist license in the State of Hawaii.
• Ability to meet Hawaii Department of Education requirements for working in a school-based setting.
• Background clearance and TB clearance as required.

Responsibilities

• Conduct formal and dynamic physical therapy assessments to determine students’ functional needs.
• Develop and coordinate treatment plans in collaboration with the multidisciplinary team.
• Provide direct Physical Therapy services aligned with students’ IEP goals.
• Participate in IEP meetings, annual reviews, and triennial evaluations as appropriate.
• Collaborate with teachers, families, and other providers to support students’ mobility, participation, and progress in the educational environment.
• Provide recommendations regarding positioning, mobility, adaptive equipment, and accommodations.
• Maintain accurate therapy notes, assessment data, progress reports, and other required documentation.
• Support students across a variety of developmental, physical, and functional needs within the school setting.
