Job summary

The Village for Families and Children in Hartford, CT seeks a Juvenile Justice Case Manager for the LYNC Program to coordinate care for youth and families in collaboration with LYNC staff and community providers.

Responsibilities include assessments, RRSP development, structured case management, discharge planning, and linking families to community services while maintaining trauma-informed, culturally competent practices.

Qualifications

• Bachelor's degree or high school diploma with relevant experience in youth services.
• Minimum of three years experience working with justice/system involved youth.
• Excellent oral and written communication skills.

Responsibilities

• Provide direct services to youth and families via coordinated care.
• Develop and monitor Risk Reduction Sustainability Plans (RRSP).
• Conduct assessments and discharge planning with community connections.
• Collaborate with LYNC staff, schools, courts, and other providers.
• Maintain accurate documentation and reports per policy.
