Qualifications

Responsibilities

• Provide care coordination and connect youth and families to community resources.
• Develop and monitor Risk Reduction Sustainability Plans (RRSP).
• Complete documentation accurately and timely per agency policies.
• Collaborate with service specialists, schools, courts, and community providers.
• Facilitate discharge planning and follow-up services.
• Travel to various sites across the Greater Hartford area as needed.
