Qualifications

• Experience with Juvenile Justice systems.
• Knowledge of Foster Care, Adoption Assistance, and related behavioral health services.
• 2 years in behavioral health, social services, or juvenile justice.
• Proficiency with MS Office Suite (Word, Excel).

Responsibilities

• Assess members using care management tools and reviews to support optimal health outcomes.
• Identify high-risk factors and plan proactive interventions.
• Coordinate Wellness care plans and monitor member needs.
• Collaborate with case managers, supervisors, and Medical Directors to improve care.
• Escalate quality of care issues through established channels.
• Use negotiation and motivational interviewing to meet member needs.
• Coach members to make informed medical and lifestyle decisions.
• Assist members in collaborating with providers in decision-making.
• Document care management processes in alignment with policies.
