Job summary

Northwestern Community Services Board seeks a Juvenile Forensic Case Manager to help youths access medical, social, educational, and other services. The role involves assessing needs, developing and reviewing service plans, and coordinating with community resources across jurisdictions.

Qualifications

• Maintains complete and accurate client records and participates in ongoing education.

Job description

The Juvenile Forensic Case Manager serves to assist juveniles in gaining access to needed medical, social, educational, and other services. The position is responsible for assessing client needs, service plan development, implementing and reviewing service plans, and working with other community resources in meeting client service needs in all jurisdictions. This position will support children and adolescents with co‑occurring diagnosis and who have court services involvement due to criminal charges or diversion.

Major Duties and Responsibilities

• Assesses and plans services, to include developing an Individual Service Plan (ISP). The case manager must modify the ISP as necessary, review it every three months, and rewrite it annually.
• Provides competency restoration services to youth identified through DBHDS; maintains compliance with documentation standards defined by DBHDS.
• Maintains close communication and works collaboratively with the Child/Adolescent Court Services Therapist.
• Ensures monthly case management contact, activity, or communication relevant to the ISP are provided. A face‑to‑face client contact must be made at least once every 90‑day period.
• Provides and links the client to needed services and support specified in the ISP.
• Coordinates and monitors services and treatment planning with other agencies and providers.
• Enhances community integration through increased opportunities for community access and involvement and creating opportunities to enhance community living skills to promote community adjustment.
• Makes collateral contacts with significant others to promote implementation of the service plan and community adjustment. Periodic home visits are expected.
• Provides education and supportive counseling, which guides the individual and develops a supportive relationship that promotes the service plan. Provide problem‑solving activities designed to promote community adjustment and to enhance an individual’s functional capacity in the community. These activities must be linked to the goals and objectives of the Case Management ISP.
• Participates in regular interdisciplinary staff meetings and continuing education.
• Maintains complete and accurate client records, in accordance with all applicable regulations.
• Meets regularly with the immediate supervisor as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues.
• Maintains close communication with the consulting psychiatrist for input regarding medication compliance, side effects of medication, and medication changes and alerts identified staff of any changes in client adjustment which might suggest decompensation and a need for more aggressive intervention.
• Performs related duties as required or deemed appropriate to the accomplishment of the responsibilities and functions related to this position or in support of departmental and agency goals.

Qualifications

• A combination of mental health work experience or relevant education, and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties required.
• Bachelor’s degree earned from an accredited college or university in a relevant human services field required.
• At least one year of experience working with courts, forensics, youth, and their families required.
• Qualified Mental Health Professional (QMHP) eligibility required.
• Must possess a valid driver’s license, a good driving record, and the ability to provide your own transportation.

Additional Requirements

• May be exposed to allergenic environments from client’s homes, including dust, smoke, pet contamination, cleaning supplies, etc.
• May be exposed to clients with infectious diseases
• Must be able to drive for long periods of time to travel in and out of the catchment area as needed
• Must be able to walk distances, climb and descend stairs, maneuver in and out of agency vehicles
• Prolonged periods sitting at a desk and working on a computer
• Must be able to lift/push/pull up to 25 pounds at times

Salary: $52,788 – $54,637
