Job summary

Journey Home in Crowley, TX is seeking a dedicated case manager to support residents at Crowley. The role focuses on maintaining housing and mainstream benefits, increasing income and self-sufficiency, and providing information, referrals, life skills training, and long-term planning.

Duties include in-unit case management sessions, needs assessments, resource identification, coordinating with services, and documenting received services.

Qualifications

• A Bachelor's degree in Social Work or related field is required.
• Experience working with diverse and/or difficult populations desired.
• Experience working with the homeless population is preferred.
• Experience working with individuals with mental illness is preferred.
• Current knowledge of the local network of services preferred.
• Knowledge of Microsoft Word and Excel is required.

Responsibilities

• In-unit case management sessions with residents.
• Assess needs and identify resources matched to those needs.
• Coordinate with residents to connect with services and document them.
• Work with all relevant departments to coordinate services in line with the mission.
• Maintain housing and mainstream benefits, and promote income/self-sufficiency.

Job Summary

Responsible for case management activities for residents residing at Crowley with the primary goals of maintaininghousing and mainstreambenefits along with increasing income and self-sufficiency.Case management includes providing information and referrals, life skills training, and long term goal planning with residents.Duties include in-unitcasemanagementsessions,assessmentofneeds,identificationresourcesmatched to the need, coordination with residents to connect with services, and documentation of thoseservices.Servicesmustbecoordinatedwithallrelevantdepartmentsandmustfall within the mission of Presbyterian Night Shelter.
