Continuum Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our team delivering exceptional end-of-life care in Sonoma and Marin counties. You’ll support patients and families with dignity, comfort, and human connection.

Responsibilities include collaborating with the Clinical Director, assessing eligibility for services, and providing direct, compassionate nursing care. BSN preferred; RN license and CPR required; hospice/home health experience welcomed.

Qualifications

• Must hold a current unencumbered RN license.
• Bachelor's degree in nursing from NLN-accredited program.
• CPR certification current.
• Hospice/home health experience preferred but not required.
• Computer skills and EMR proficiency.

Responsibilities

• Respond to clinical referrals with care and expertise, addressing inquiries compassionately.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct nursing care to maintain patient comfort and quality of life.
• Deliver nursing care rooted in proven principles and techniques.
• Educate patients and families to participate in personalized care plans.
• Share call duties as part of the team.

Continuum Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR
• One (1) year experience as a professional nurse within the last three (3) years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Compensation

$64.00 - $66.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate's relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
