Job summary

Minnesota Diversified Industries in Cohasset, MN is seeking a Job Coach to provide ongoing training support to employees and contractors with documented disabilities, helping them meet supervisor expectations.

You will model skills, deliver coaching, document progress, and ensure compliance with 245D and licensing requirements while maintaining confidentiality and a safe work environment.

Qualifications

• One year experience delivering supported employment services or direct care.
• Ability to read, write, recognize, understand and accomplish new assignments with only general instructions.
• CPR and First Aid Certification.
• DHS Background Study and a criminal background check clearance.

Responsibilities

• Model appropriate work skills consistently.
• Provide enrichment training, support and coaching to participants.
• Work with supervisor to introduce natural supports and provide prompts as needed, and ensure staffing ratio.
• Document employee goals and daily activities in Pavillio or required company software.
• Uphold and consistently follow 245D program and all licensing requirements.
• Maintain and model positive relationships.
• Demonstrate knowledge of disabilities of participants and provide quality service per Support Plan Addendum, IAPP, and SMA.
• Meet mandatory training requirements.
• Ensure compliance with work instructions and/or QMS procedures.
• Adhere to all organizational policies.
• Function in a fast paced, diverse, team environment.
• Maintain ethics and confidentiality.
• Maintain a safe work environment including wearing PPE.
• Assist in monitoring bus for employees using public transportation.

Job Description Summary

Essential Functions

• Consistently model appropriate work skills.
• Positively provide enrichment training, support and coaching to participants.
• Work closely with supervisor or line lead to introduce natural supports, provide prompts and feedback as needed, and ensure staffing ratio
• Document employee goals and daily activities in Pavillio or required company software.
• Uphold and consistently follow 245D program and all licensing requirements.
• Maintain and model positive relationships.
• Demonstrate knowledge of disabilities of participants and the ability to provide quality service to them in accordance with the employees Support Plan Addendum, IAPP, and SMA.
• Meet mandatory training requirements.
• Ensure compliance with work instructions and/or Quality Management System (QMS) procedures.
• Ensure adherence to all current organizational policies.
• Functions effectively in a fast paced, diverse, team environment.
• Maintain highest levels of ethics and confidentiality
• Maintain a safe work environment for self and others including wearing mandatory Personal Protective Equipment (PPE).
• Assist in monitoring the bus for employees required to take public transportation to and from work.
• Understanding of Lean Manufacturing and key principles of efficient productivity.
• Other duties as assigned

Qualifications

• High School diploma or equivalent preferred but not required.
• One year experience delivering supported employment services or direct care services.
• Ability to read, write, recognize, understand and accomplish new assignments with only general instructions.

• Have or obtain CPR and First Aid Certification
• Cope with emergency situations in a calm and efficient manner.
• Able to communicate effectively with program participants.
• Satisfactory completion of a DHS Background Study and a criminal background check.
• Verified eligibility for employment in the United States.
• Valid state-issued driver's license.

Environment and Physical Demands

• A traditional work week (M-F) with MDI core hours. Work non-traditional hours periodically when required.
• While performing the duties of this job, the Job Coach will be required to use hands and fingers to handle or feel; reach with hands and arms; speak and hear clearly. The noise level in the work environment varies low to high. The jobs requires the ability to walk, climb stairs, kneel, stoop, and may require employee to lift and/or move items weighing up to 50 lbs. Specific vision abilities required by this job include close vision, color vision, distance vision and ability to adjust focus. The job may require extended periods of sitting or standing.
