Job summary

Hill Country Mental Health and Developmental Disabilities Centers seeks a Care Coordinator for the Jail-Based Competency Restoration (JBCR) program. You will assist with daily curriculum, case management, and interaction with individuals enrolled in JBCR, while coordinating with jail and court staff.

You will provide forensic instructional programming, document progress, monitor competency restoration, and support discharge planning.

Job description

The Care Coordinatorassists with daily curriculum, provides case management services, and interactswith people enrolled in the Jail-Based Competency Restoration (JBCR) program.Assists with administrative and communication with court and jail staff. TheCare Coordinator will provide forensic instructional programming, documentindividuals' progress, monitor individual progress toward competencyrestoration, transition services, and discharge planning. The Care Coordinatorwill report any difficulties the individual may be experiencing to supervisorystaff. Care Coordinator will work collaboratively with jail staff, court staff,and administrative staff. The Care Coordinator will also be responsible for dailyaccurate clinical documentation, which will be shared with the court. The CareCoordinator will also conduct other programming with individuals such as grouptherapy, wellness exercises, and art therapy, and helpformulate other programming. They will also oversee and coordinate dischargeplanning and coordination of further services as necessary.

You will get to:

  • AssistwithGroups and Educational curriculum.
  • Assist with the provision of legaleducation training and psychosocial skills training thataddresses their competency to stand trial and their symptoms related to mentalillness and/or substance abuse issues.
  • Providetraining/education with jail staff on the nature of mental illness as outlinedin Comal and Hill Country curricula under the supervision of the ProgramManager.
  • Communicate withJBCR Educators and all staff in the program about persons served.
  • Assist withadministrative support for the JBCR Program Manager in accordance withdepartmental/organizational procedures.
  • Monitor consumerprogress with treatment goals.
  • Maintain the flowof information for court orders using multiple systems and effectivelycommunicate with judges, attorneys, and other court staff as needed.
  • Attend andparticipate in team meetings with the clinical team.
  • Work well withothers in a collaborative manner.
  • Activelyparticipates as a member of the team.
  • Comply withHill Country and Comal SO policies and procedures, accrediting/regulatorybodies, and contractual requirements.
  • Demonstratesethical conduct, professionalism, and confidentiality in all activities andworks with staff/patients/outside personnel in a manner that demonstratessensitivity to cultural diversities.
  • Adapts tochanging work requirements.

You'lldefinitely need:

  • Bachelor's degree from an accredited college or university in social, behavioral, or human services with the ability to be credentialed as a Qualified Mental Health Professional (QMHP-CS)
  • Maintain a valid Texas state Driver's License and auto insurance.
  • To complete and maintain both CPR certification and Prevention and Management of Aggressive Behavior training within the first 3 months of employment

  • Must be able toprioritize and plan work activities to use time efficiently; can manageseveral projects and tasks simultaneously.
  • Able to meettight deadlines in time-sensitive situations while maintaining a professionaldemeanor.
  • Must bedependable, able to follow instructions, respond to management direction, andmust be able to improve performance through management feedback.
