Job summary

Hill Country Mental Health and Developmental Disabilities Centers is seeking a Mental Health Care Coordinator to assess needs, develop person-centered recovery plans, and collaborate with individuals and families. You will provide skills training, crisis prevention/management, and referral to support goal progress for children, adults, and families with mental health challenges.

• Valid Texas driver's license and auto insurance.
• CPR certification and PMAB training within first 3 months of employment.

Responsibilities

• Assess individual needs and collaborate to develop recovery and life management plans.
• Provide skills training to improve daily life.
• Collaborate with service providers and agencies to coordinate care and outcomes.
• Write and document articulately with correct grammar for clear records.
• Maintain compliance with legal requirements and company policies.
• Meet with individuals regularly to monitor progress and compliance.
• Be available to manage crisis situations as they occur, sometimes off-schedule hours.
• Employ excellent verbal and written communication to engage and report effectively.
• Keep a well-organized schedule with flexibility for adjustments.
• Maintain accurate, timely collaborative documentation.
• Remain calm under pressure to meet deadlines and handle difficult situations.
• Provide exemplary customer service and care with client best interests at heart.
• Work autonomously to complete assignments within company and state parameters.
• Become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification.

A Mental Health Care Coordinator will assess needs,develop treatment plans, and effectively collaborate with individuals and theirfamily members. The Care Coordinator will assess strengths and needsand, in collaboration with the individual, develop a person-centered recoveryplan. The Care Coordinator will provide skills training, crisisprevention/management, linkage, and referral to support goal progress forchildren/adults and families living with mental health challenges. Thiscompassionate, caring, and supportive team member will contribute to the skillsimprovement, achievements, and overall potential success of a child/adult and theirstate of well-being.

You willget to:

• Assess individual needs andcollaborate with them to develop and execute effective recovery and lifemanagement plans.
• Provide relevant skills trainingto improve daily life.
• Collaborate with serviceproviders and agencies to be a liaison for effective care and positiveoutcomes.
• Write and document articulatelywhile using correct grammar so that documentation is clear and valuable.
• Maintain compliance with alllegal requirements and company policies and procedures.
• Consistently meet withindividuals to maintain progress as well as compliance.

You'll definitely need:

• Bachelor's degree from an accredited college or university in social, behavioral, or human services with the ability to be credentialed as a Qualified Mental Health Professional (QMHP-CS)
• Maintain a valid Texas state Driver's License and auto insurance.
• To complete and maintain both CPR certification and Prevention and Management of Aggressive Behavior training within the first 3 months of employment

Preferred -

• At least one year of experience implementing the Child and Adolescent Needs and Strengths Assessment or the Texas Resilience and Recovery Model for Adults or Children.

• Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter.
