Job summary

Community Bridges, Inc. (CBI) is seeking a Jail Coordinator to deliver treatment and discharge planning for County Jail Program Patients and assist inmates with jail release planning and transitioning into behavioral health services.

Qualifications

• Highschool diploma or GED required. Associates degree or higher preferred.
• Experience in a criminal justice setting, behavioral health, physical health or substance abuse setting required.
• Minimum of six months of recovery from substance use and/or mental health disorders preferred.
• Fingerprint Clearance Card preferred.
• Must be able to pass a background check and receive clearance to go into the County Jail.

Responsibilities

• Deliver treatment and discharge planning services for County Jail Program Patients.
• Assist inmates with jail release planning and coordinating into behavioral health services.
• Participate in a weekly staffing with probation and jail staff to provide updates on each patient and coordinate care.
