Job summary

Community Bridges, Inc. (CBI) is seeking a Transition Coordinator-Residential to manage individualized discharge plans across care levels and to ensure timely follow-up appointments within 7 days of discharge.

The role coordinates with Utilization Management for authorizations and delivers counseling and discharge planning to patients and their support systems. The ideal candidate has a high school diploma or GED (an associated Behavioral Health degree is preferred), 2+ years of health care

Qualifications

  • Highschool diploma or GED required.
  • Associate’s degree in a field related Behavioral Health preferred.
  • Minimum of 2 years of full-time health care related work experience with exposure to behavioral and medical situations.
  • Experience with administrative or operational processes in a health care setting preferred.
  • Experience working with specialty populations (SUD, SMI, children, adolescents) preferred.
  • Bilingual candidates preferred.
  • Valid identification required.
  • Behavioral Health Technician (BHT) per policy is required.
  • Fingerprint clearance card required.

Responsibilities

  • Coordinate discharge planning for patients across care levels within CBI systems of care.
  • Schedule follow-up appointments within 7 days of discharge.
  • Coordinate with Utilization Management on authorizations and continued authorizations.
  • Complete initial assessment, treatment plan, and engage patient and support system.
  • Provide therapeutic services including counseling, group work, discharge planning and authorization coordination.

Transition Coordinator-Residential

Community Bridges, Inc. Job Summary

Transition Coordinators are assigned at each level of care within the Community Bridges, Inc. (CBI) systems of care network to ensure that patients/patients have an individualized discharge plan. All follow up appointments for ongoing care and services will be scheduled within 7 days of discharge. Transition Coordinators are responsible for coordinating with Utilization Management on all authorizations and continued authorizations. Transition Coordinators will adhere to all policies and procedures. The Transition Coordinator will complete an initial assessment, and treatment plan, as well as engage the individuals and their support system. The Transition Coordinator will provide therapeutic services to individuals and their support systems to include counseling, group work, discharge planning and authorization coordination.

Skills/Requirements:

  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures, is required.
